Medicare Advantage Clinics

We’ve developed a new model of healthcare to meet the needs of our Presbyterian Medicare Advantage plan members. Our Medicare Advantage Clinics are dedicated to prioritizing timely care, offering extended appointments, and providing a personalized approach to enhance the well-being of your healthcare.

Our providers offer primary care focused on the needs of adult patients and will lead your healthcare journey and be an advocate for your health concerns. They provide annual check-ups, health screenings and education, immunizations, and orders for lab work or radiology, as well as manage complex or chronic health conditions, treat an urgent illness or injury, and provide referrals to other specialists, including any required prior authorizations.

These clinics only accept Presbyterian Health Plan Medicare Advantage members.

Conditions Treated

A few common conditions treated include but are not limited to:

  • Respiratory infections, including bronchitis or pneumonia

  • Adult to elderly care concerns

  • Chronic health conditions, such as high blood pressure or diabetes

  • Bone and joint care

  • Allergy and asthma care

  • Non-chronic wound care and non-complex diabetic foot care

Testing & Treatment Options

A few common tests and treatments include but are not limited to:

  • Coordination of care for chronic health problems

  • Treatment of an urgent illness or injury

  • Diagnostic testing

  • Annual check-ups, health screenings and immunizations

  • Preventive health recommendations

  • Referrals to specialists, as needed

Our Providers

Our primary care providers focus on the health and well-being of adult patients. As primary care providers, they can diagnose and treat a broad spectrum of health problems, including complex or chronic diseases, as well as annual check-ups, health screenings, immunization management and nutrition recommendations.

Our Medicare Advantage Clinic is a specialized clinic that takes senior healthcare to another level. We focus on offering longer appointments so you can have all your questions and concerns addressed before you leave. This includes referrals, prior authorizations that may be required, and communication with any support services.

Our appointment times range from 30 minutes to one (1) hour. If you need extra time, we will be able to accommodate.

Typically, you can get your first appointment within two (2) days of requesting one.

We value your time and aim to provide same day services for a variety of situations. Often people visit the ER or urgent care for an issue that can be addressed by their PCP. During clinic hours, our nurse will triage your call and collaborate with the team to determine which care option is appropriate. This includes in-person same day visits, a telephone call, a message through MyChart, or a recommendation to proceed to the ER if that is clinically appropriate and necessary.

Yes. If you have been treated at a Presbyterian facility before, we will have access to your records electronically. You will also still have access to your records via MyChart and will retain the ability to schedule online. If you have never seen a Presbyterian provider, we can work with you to obtain your medical records.

We expect you to be fully satisfied with your new experience but want to assure you that if you are dissatisfied with the clinic or a provider, we will work with you to find a new PCP.

The relationship one has with their PCP is an essential piece to your overall health. Our providers’ approach to patient care is built on trust, collaboration, and respect. They will dedicate your visit to listening to what is most important to you and serve as an advocate to help you achieve your best health.
